Food Concession - Round Up for Summer Entry Forms

Non-Profit Organizations Only - DEADLINE IS FRIDAY, APRIL 30, 2012

Booths will be approximately 12’ x 12’. The fee is $25.00 and must be sent with this application, plus 10% of your actual sales. We will be doing this on the honor system and expect each booth to keep track of their sales and send a check to Winfield Main Street for the 10% within one week after the event. 

In addition to submitting the form, you must pay the appropriate fee via our online store.
